Tele-Triage Licensed Practical Nurse - LPN

Starting Point Nursing Services offers a part-time tele‑triage role for licensed practical nurses who hold a compact state LPN license from their current residence. The position is fully remote and scheduled for evening shifts on Wednesday, Saturday, and Sunday from 10 p.m. to 6 a.m. CST (11 p.m. to 7 a.m. EST). The LPN will primarily conduct on‑call triage of symptom‑based calls, assess patient severity, and provide recommendations per established protocols. Responsibilities include reviewing charts, completing monthly home‑visit reviews, performing outbound calls to assigned populations, and conducting data analysis across multiple systems. The nurse will collaborate with operations LPNs to develop and update care plans, coordinate discharge planning with acute‑care settings, and manage medication orders and prior authorizations. Communication with beneficiaries will occur via phone, text, chat, or email, and the role requires strict compliance with state, local, and payer documentation standards. Documentation duties involve updating the Nurse’s Notes, Service Delivery Log, and time‑sheet records.
